Custom Form Integration

Hello,

I would like customers to complete a custom registration form, instead of just the standing billing and shipping information. This form would have conditionals etc.. My team is using .NET 2.0 and I'm hoping we can somehow create our own gateway and add these form elements to that. I understand that we would need to create the backend tables, etc..., but my question is: Is this the best method for doing so, and if so how do we go about creating a custom gateway, along with adding this form? If not, what are the best practices? Note: I did find the custom product fields, but they don't seem to be a solution for this.

Any help would be very much appreciated.
