Randomly Orphaned Items

Hello... just wondering if anyone could shed light on something for me...
It seems that randomly, without editing products or categories, we will have certain items just show up in the orphaned items table.
Any ideas why this might be occurring?

I have experienced this as well. I would just click on the Orphaned Products link and would see products there without making any changes to the site. Now I usually click on it daily to see if any products show up. It hasn't happened to me again within the last couple of weeks.

In development we had some randomly show up.

After we went live we were strict with who has access to the Catalog. Haven't had an issue since.
