Customized shipping charges?

Hello,
I have an unusual situation.
For some of the items in our store, the shipping needs to be "quoted" after we calculate the shipping charges by hand.
Is there any method by which we have the customer construct (but not finalize) an order, until we set the charges for that order?
My thought is that an Admin person can place a custom-priced item (the special shipping) into the customer's cart, after which they can complete the checkout?

I hope this makes sense.

You could do that with a fake product allowing a variable price and the admin could add it to the order and fill in the price. You could make it a hidden product.

Would you need to use a different payment method then standard credit card. So that the order is not run until the shipping is added. Or should you approve the card for the product amount. And run the card for the final amount?

I was thinking about that. I'm not sure how to prevent the order from completing before we add the customized shipping charge.
