Extra shipping charges

It Appears the cart is keeping track of shipping as an item maybe?

If a client clicks on next day it puts it in the cart even if they go back to selecting ground, i think it is accumulating the charges rather than selecting only the last one chosen?

has anyone else had issues with Duplicte shipping charges?

One of my clients has been having a problem, but he thought it was because of customization code someone had done. I think he took off the custom code, but he never could track what was causing the problem - customers couldn't remember exactly what they had done.
He is running build 10863.

Shipping is stored as a hidden line item on the order.

Surely someone would have found this by now if it were part of the base AC7 install......that build has been out for many months hasn't it?
