I am new to AC. Is it possible to add an input box for a middle initial in the user profile area? We are finding that we get several of the same names such John Smith. So we want to be able to tell them apart more easily.

In above thread Nickname field is being exposed to OnePageCheckout. You can do very same one profile page as well.
