Adding a simple poll/vote to a category page

We want to add a simple vote to either a category page or a custom page on our site. The vote will be a simple single question with likely three available choices (as radio buttons, preferably). It would also be nice to show the running percentage for each response once a customer votes.
I did not see something built into AC7 to do this, and wonder if someone has something similar that they can share, or can suggest an off-the-shelf option. What I have seen available is not really something that can be incorporated into our current website pages.
