Shipping not calculating

I just had a weird order in which shipping did not calculate and the customer got free shipping. In fact, a shipping method was not even selected or used. There were 2 products on the order. Each of these is configured properly with weight and warehouses. I sell these products all the time and have not had this problem.
Anybody have any ideas on why this would happen?

We've had the same problem with two of our sites. They are running 7.0.6. The problem hasn't happened frequently enough for us to find the steps to recreate the problem (8-12 times over the past 12 - 18 months). The client is getting anxious for us to fix it. Our next approach was going to put an additional check into the cart page that would send them back to the beginning of the checkout process if there was no shipping record for the order (we don't have any non-shippable products).
We hope to do this next week. We will have to wait a couple of weeks before we can determine if our change was successful. If we don't see the problem after a couple of weeks we'll post our change.
We would appreciate knowing if you find a problem.

I have the same issue but it seems to only be one customer that it has happened to a couple of times.. I have not really looked into it.
I remember back in the day that AC was changed to make sure no order could go through without shipping but evidently there is still some cases where it can happen.

We have it happen occasionally and have never been able to track the issue down.
