A customer wants to exchange an item that did not fit and wants us to send her a different size. Is there a way for us to charge her for shipping within the same order or would we have to place a new order under her name. (The only thing we want to charge the customer is shipping)

There is no ability to add shipping to an existing order once the order is placed.
You can either add a new line item using the Order Items button on the order details page or place a new order.
When that happens to me, I add three line items to the existing order.
One that is qty -1 to represent the item returned. Charge is original charge.
One that is Qty +1 different SKU to represent the replacement item. Charge is 0.
One for the replacement shipping charge. Charge is whatever you charge her for shipping.
