Issues after build upgrade and patches

Last night we upgraded AbleCommerce 7.0 to build 9879. We then applied the patches that brought the build up to 10863.
We now have two problems - one with issuing credits and one with the product name on the sales receipt.
When we try to issue a credit we get the following error message

SUCCESS
Transaction ID:2192454344
AVS:Unavailable (U)
CVV:No Response (X)
11/26/2008 9:28 AM
Authorize.Net Refund $21.21
FAILED
Our sales slips are also now printing the Product Template information in the Name section instead of the product name
Please advise what steps we need to take to correct these two issues.
