I need a developer to do the following.

6. Add new section (below billing address area) called "HOW DID YOU HEAR ABOUT US?" which requires the user to choose one of the following:
How did you hear about us?
Doctor
Newspaper or Magazine
Friend or Family
Web Search
Other (please specify): Text Field
6.a) if "Doctor" option above, is checked, the user should be required to choose further sub-options (ideally revealed as a show/hide drop-down that appears only when "doctor" is selected). Ideally, the required fields for each of these sub-options would themselves also show/expand only when the relevant option is selected, if possible:
How would you like to identify your doctor:
By PIN number: text field allowing 7 characters
(sometimes provided by doctor or printed on a prescription sheet)
By doctor's last name and phone Dr. : text field for name Phone:
By doctors' last name, city and state Dr. : text field for name City: State
